NEW ENQUIRY/REFERRAL FORM
How it works
01
REFERRAL
To get started, we’ll ask you a few questions to gather details about your child’s needs, through our Referral Form found bellow. Providing as much information as possible helps us tailor our support to your family. Next, Dianna will call you to discuss your responses and gain a clear understanding of which service will suit your child best. Once we have a thorough picture of how we can assist, we’ll guide you through the intake process and ensure you’re well-prepared to support your child on a positive and enjoyable journey.
03
COMMENCEMENT
All therapeutic sessions are usually between 45 minutes in duration, and this may depend on the age of your child/young person and their individual needs, as well as the needs and treatment goals identified along the way. They ideally occur on a weekly basis and take place on consistent days and times to ensure predictability and prioritisation of therapy.
02
INTAKE
The intake session is our first step together and serves as an initial consultation for our services and support. This one-on-one meeting, just between you and Dianna, typically takes around 1-2 hours. Ideally, we ask that children not attend, as it’s a space where we can openly discuss important matters, giving you the chance to share your thoughts honestly and openly.
In this session, we’ll have a genuine and thoughtful conversation about what’s happening for your child, covering all aspects of your family’s life. We’ll look into early life experiences, development, and connections that have shaped your child. This time together helps us understand the best way forward and what a treatment plan might involve. Our approach is truly collaborative, valuing the knowledge and insights of parents and caregivers as central to the therapy process.
04
REVIEW SESSIONS
Involving parents and caregivers is essential when working with children under 15. To ensure privacy and open communication, we set aside dedicated time to connect with you without your child present. These 50-minute parent consultations allow us to check in, address questions, and track your child’s progress together. This approach keeps you fully informed and actively involved in your child’s therapeutic journey. Typically, these sessions are scheduled every fifth visit, but we can adjust the timing to suit your family’s needs.
